The Family Promise aftercare program designed to secure long-term stabilization for families that have graduated back into independent housing.
Housing stabilization is our aftercare program for client families that have graduated from the shelter program into independent or transitional housing units. It focuses on facilitating long-term housing sustainability through mentorships, monthly case management check-ins, and occasional provision of emergency financial assistance.
The goal of the program is to eliminate the chance for clients to relapse into homelessness. It has proved imperative to the success of FPNFD client families. In 2021, the eight families that graduated to independent living maintained their independence for over 14 months.
Our goal is for every graduating family to succeed!
