Aftercare Program: *12 Months of Continued Support
Moving into housing is a major milestone, but long-term stability doesn't happen overnight. Family Promise's Aftercare Program provides continued support to families after they graduate from our shelter and housing programs.

🏡 Ongoing Support and Case Management:

Families remain connected to their case manager for up to 12 months after graduation, receiving guidance, accountability, and support to help maintain stable housing and continue progressing toward their goals.
💵 Financial & Credit Coaching

Families continue to receive support with budgeting, financial education, credit support, and long-term planning.
🔧 Emergency Assistance

When unexpected challenges arise, such as car repairs, utility emergencies, or other urgent needs, we may be able to provide assistance to help families remain stable.
🌐 Community Resources & Referrals

Families remain connected to employment resources, community partners, and other essential support services.
